Summer & School Year Health and Wellness Requirements
 
PHYSICALS:
All rising 6th-12th grade students are required to obtain a physical & complete the Pre-Participation History & Health Assessment prior to August 1, 2020, regardless of whether the student is playing a sport.
 
If the student plans to tryout, practice, or participate in HHCA athletics over the summer, they must have a physical dated on or after May 15, 2020, on file within their Magnus account before participating in any training. 
 
To help facilitate this, HHCA is providing two onsite opportunities for students to receive their physical exam at a minimal charge of $20.  To adhere to recommended social distancing guidelines and provide quicker service, students must make an appointment and have all forms completed prior to arriving. Thank you for arriving promptly for your scheduled physical and honoring proper social distancing at all times for the appointment.

HHCA ON-CAMPUS PHYSICAL CLINIC:
Thursday, May 28th (complete) AND Friday June 5th | 9 AM-12 PM          
 
The following items must be completed prior to your child's scheduled appointment for the physical to be completed.
  1. CLICK HERE to be directed to myHHCA where you will find the Magnus Health Portal login in a blue box.

  2. Login to your child's Magnus account and print the Pre-Participation History & Health Assessment Form. Complete this form and upload it to your child's Magnus account (do not upload the form to be signed by the physician, please bring it with you to your appointment).

  3. Complete & sign all additional electronic forms (NOTE: parent and student should sign each electronic form).
     
  4. If the student is having their physical performed at HHCA, we must have all forms signed and uploaded to Magnus by the parent. Payment of $20 cash or check payable to HHCA will be due at the time of the appointment.

  5. If student is not having their physical performed at HHCA, we must have all forms, including physician signed physical, uploaded to Magnus by parent no later than June 5, 2020 for athletes or August 1, 2020, for non-athletes.

IMMUNIZATIONS:
If you have not already done so, please make sure your child's immunizations are up to date for next year.  ALL students must have current and up to date immunizations in order to return for the first day of school next year.  Please refer to attached files below for more information. 
 
Updated immunization records are due by August 1, 2020; please upload them to your child's Magnus Account.

If your child is a rising 7th-grade student, please be sure they have the Tdap vaccination prior to August 1
, 2020.  This is a state regulation - children without this booster may not be allowed to attend school for the 2020-2021 school year. Tdap vaccinations are required in addition to the DTaP and DT/T vaccination. Immunization records can be uploaded directly to your child's Magnus account or scanned and emailed to wcummings@hhca.org.
 
NEW State Immunization Requirements for School Attendance:

*All children starting 5K in 2020 will need to have two (2) hepatitis A shots.                
*All rising 4th-grade students must have two (2) doses of varicella. A child with a positive history of the disease is considered immune and is exempt from this requirement.
*All rising 6th-grade students must have three (3) doses of oral and/or inactivated Polio vaccine with at least one (1) dose received on or after the fourth birthday. 
*All 12th-grade students must have one (1) Tdap (tetanus, diphtheria, and pertussis) booster. Tdap is routinely administered at 11-12 years of age; however, a dose administered on or after the seventh birthday will meet this requirement. **Reminder that students must have a Tdap booster on or after the 7th birthday.

OTHER HEALTH UPDATES:
  • Student Medication: - Any medications from the 2019-2020 school year will be returned along with student belongings during "Drop off/Pick up Day".

  • Electronic Health/Magnus Forms:  Please take a moment to review and electronically sign all the other health forms on your child's Magnus account, making sure any information that may have changed over the course of the year has been updated.  All forms must be signed and dated for the 2020-2021 school year.  You can access your child's account throughout the summer and school year to make any necessary changes or additions.  

  • Personal Info & Address Changes:  Please be sure that if you have changes to your personal information such as address, phone number, etc., that you alert the front office of that change as well.
