ON-CAMPUS SPORTS PHYSICALS 

April 22-23, 2026 | For Rising 6th-12th Grade Student-Athletes

HHCA will once again offer on-campus sports physicals during the school day on Wednesday, April 22, 2026, and Thursday, April 23, 2026. Rising 6th-12th grade students planning to participate in Middle School or Upper School athletics during the 2026-2027 school year, including any summer training or conditioning, are invited to participate.

A current sports physical dated after April 1, 2026, is required for athletic participation (which may begin over the summer). This on-campus event provides a convenient way to complete that requirement before next school year.


📌 Important Registration Information

  • Pre-registration is required. Parents must complete the registration and payment form (available soon on this page) in order to participate.
     
  • After submitting the registration form (which will open soon on this page), you will receive a confirmation email containing a link to choose your student’s appointment time.
     
  • Students must bring this required form to the appointment with the medical history page completed and signed by both student and parent.
     
  • Appointment times are available on a first-come, first-served basis.

📌 Magnus Health Reminder

In order for students to be eligible to participate in athletics during the 2026-2027 school year, the completed physical form must be uploaded by the parent to the Magnus Health Portal.

  • Paper copies will not be accepted

  • HHCA staff will not keep a copy on file

  • Students may not participate until the completed form is uploaded to Magnus

*HHCA is not responsible for lost paper forms after the appointment, so please upload your student’s completed form as soon as possible.
